It has a conception of “this is what a man is” and “this is what a woman is,” and those things have very little overlap. For instance, every man from an overworked salaryman to a homeless man to an otaku can fight, but the only women who ever fight are the ones in your party, because even when a woman is ordering armed goons to kill you, it would be wrong in this worldview for your heroes to beat up a woman. (My understanding is that this is the first game in the main canon where female player characters even get combat encounters.) But it’s not 100% straight, because the male/female relationships don’t go as hard on the sexual undertones as some of the other games I’ve played.
The only other Yakuza I’ve played is a remake of the first game. In that one, you beat up a trans man, a trans woman, and a creepy crossdressing guy who seems to want you to stick your dick in him, but also seems to view his knife as a metaphorical dick he wants to stick in you. You never beat up a cis woman. I have no idea what that says.
