I’m weirdly resistant to body horror, but only if it’s fantastical. Guy’s chest splits open with an axe? I cringe away from the screen. Guy’s chest splits open into a hideous fanged maw? Eh, whatever. Some design choices bother me more than others, like bloodied fangs or holes that look like larva hatched from them, but none of it freaks me out nearly as much as straightforward violence.

 I read a few chapters of Girl That Can See It when it was first translated. I thought all the fantastical body horror was mildly perturbing. I’ve just seen some pics from later chapters, and what the flying fuck is going on in this manga? (On the flipside, Mortal Kombat takes straightforward mutilation so far past any point of realism that it doesn’t even bother me.)

I'm guessing this is because realistic injury seems more "real" to you; it's something that might actually happen. Also, a lot of fantastic body horror is transformation, and it's not obvious that transformation would hurt in the same way injury would.
